Novel skin rejuvenation procedures featuring newer, more advanced technologies are entering the aesthetic marketplace in greater numbers. A leader in this area, Cartessa Aesthetics (Melville, N.Y.) offers the ”Ultimate Duo” procedure, which is the exclusive combination of its VirtueRF microneedling system with CoolPeel, an extension of its Tetra CO2 laser system. Practitioners report this treatment attracts patients of all ages because of improved efficacy and minimal downtime.
”There is a pent-up demand for this type of advanced skin treatment,” stated Monica Bonakdar, M.D., an aesthetic laser surgeon in Newport Beach, Calif. ”The Ultimate Duo protocol does a great job of remodeling the skin, and the VirtueRF and CoolPeel systems work well together. Patients today want minimal downtime. They don’t have time for multiple treatments when a single-session combination treatment is available.”
The Ultimate Duo stimulates collagen production at both the superficial and deep levels of the skin. First, the VirtueRF’s handpiece uses robotic precision delivery (RPD) to deliver 36 gold-coated needles into the skin, resulting in evenly distributed heat energy at the exact depth and pulse duration. The insertion process is so exact that it creates no tearing or trauma, unlike other RF-based microneedling systems that can scratch the skin, causing pain, blood, and sebum in the channels. ”The robotic component offers a very elegant withdrawal of the 36 needles,” said Sarah Gee, M.D., F.A.A.D., a dermatologist at Austin Skin (Austin, Texas). ”Patients that had treatments from other RF microneedling platforms have said the VirtueRF treatment is far more tolerable and the treatment seems to go faster. The robotic delivery seems to improve patient tolerance.”
The CoolPeel is an extension of the Tetra CO2 laser system, which features H-pulse, a proprietary high-peak-power short pulse that delivers superficial skin resurfacing results without the persistent, damaging heat associated with other laser-based systems. The combined anti-aging procedure addresses sun damage, fine lines and wrinkles, age spots, enlarged pores, skin laxity, and overall skin tone and texture.
”The risks of traditional CO2 are high, with demarcation lines and infection risks,” noted Dr. Bonakdar. ”For the Ultimate Duo procedure, we apply fractional CO2 using the Tetra laser in CoolPeel mode. It allows us to manipulate the energy delivery so that we don’t get a lot of collateral damage or heat to the surrounding tissue.”
Both devices are ”very user friendly,” stated Dr. Bonakdar. ”I have programmed the Tetra laser so that a simple push of a button brings up the CoolPeel settings I want. Also, the VirtueRF is easy to learn and understand. The only accessory between both systems is a single-use microneedling tip for each patient when using the VirtueRF. The robotic arm delivers gold-plated needles with extreme precision, making VirtueRF different from other systems on the market. Also, the system uses Chevron circuitry, so you don’t get hot spots on the skin. Cartessa has picked the best of the best technologies.”
The Ultimate Duo’s pain-free, reduced downtime procedure attracts both young and older consumers looking for skin rejuvenation, expressed Dr. Gee. ”The different demographics have different needs, but between these two devices you can satisfy a lot of the cosmetic concerns of all age groups. I have some younger patients who get this treatment that don’t really have a lot of soft tissue volume loss, but they have acne scars. The treatment helps to improve the scarring and also keeps their collagen production elevated. They like getting a superficial treatment that can also reduce pore sizes and photodamage.”
Both doctors noted that patient satisfaction is very high. ”My patients are very happy with the results,” said Dr. Bonakdar. ”Depending on the patient’s expectations and the health of their skin, I usually sign them up for a series of three treatments three or four weeks apart. After they finish the third treatment, I usually suggest one treatment once per year to maintain their skin health.”
As reported by Dr. Gee, ”The feedback has been really positive, especially with the series of three treatments one month apart. Most patients are achieving satisfactory results. I actually got this treatment myself one week ago. You really can’t look at me and see that I got the treatment. The Ultimate Duo provides satisfying results without considerable downtime, which is what our patients want. Minimal downtime is acceptable to them. You don’t want to offer something with zero downtime because zero downtime usually means less clinical efficacy.”
Speaking of good feedback, Dr. Gee said her dealings with Cartessa Aesthetics ”have been extremely positive. The people are down-to-earth and ethical. More important, they sell really great products that are elegant with good delivery systems. They might be smaller than other aesthetic device companies but their products are superior.”
Cartessa Aesthetics imports a diverse portfolio of systems from a range of manufacturers, and then recommends and brands ways to combine modalities for clinical and financial upside. ”The products they bring into the U.S. offer the latest technologies and provide excellent results,” added Dr. Bonakdar. ”For example, the Tetra and the VirtueRF were developed by different manufacturers, but Cartessa made great choices. They sure work well together. I’ve had really positive interactions with Cartessa. They are very physician-centric and focused on customer success.”
